  This questionnaire is designed to gather information on team effectiveness. Each of the 17 questions addresses specific characteristics of high performance teams. No individual responses will be provided to anyone outside of QLS; however, collective results of the survey will be used to identify areas in your organization needing improvement.

To begin the questionnaire, please provide your e-mail address and indicate whether you are a Government or contractor employee. This information is for Quantum Leadership Services use only. Again, your individual responses will not be provided to anyone in your organization. Provide a rating for each of the 17 characteristics. Your rating for each characteristic consists of two parts:

First, check the round radio button on the scale from one to nine you feel most closely resembles where the team stands right now. The three descriptors above each of the scales are there to assist you in determining a numerical score. Second, indicate if you believe each characteristic has gotten worse, stayed the same or improved over the last six months. Finally, there is an opportunity for you to provide specific comments if you desire at the end of the questionnaire.
